Reviewing a Low Assessment Result
A low assessment result calls for reviewing outcomes and gaps. COS does not enforce a general “negative assessment means return to the previous phase” lock.
Identify what you are seeing

| Low score / level | Review the model, answers, scope and missing data |
| No data / not evaluable | Check missing measurements, undefined criteria or answer status |
| Editing unavailable | Check the role, assignment, assessment/cycle status and company suspension |
| Save error | Follow the displayed validation or permission message |
Validate the result
- Check that the company, cycle and assessment are correct.
- Check which template/version and scope produced the result.
- Review your assigned questions through Goals & Tracking → My Tasks.
- Check answers, evidence and pending approvals using the relevant assessment guide.
- Follow observations in Findings as permitted; connect the relevant ones to SWOT and focus areas.
A low result and a write lock are different
Permission to reopen an assessment or change answers depends on its type and status. Use the allowed actions; you do not have to open a new planning cycle or change the mission to change a result.
KPI success is a separate issue: “criteria undefined,” “not evaluable” and a low success level mean different things. See KPI Tracking.
